1. Understanding Uterine Fibroids in Ayurveda

Uterine Fibroids (known medically as Leiomyomas or Myomas) are non-cancerous growths in the uterus that affect many women during their reproductive years.

 

Uterine Fibroids are often correlated with Garbhashaya Granthi (a knot or lump in the uterus) or sometimes as Mamsaja Granthi or a form of Arbuda (non-malignant tumor) in the reproductive tract.

 

The Ayurvedic perspective focuses on the root cause—an imbalance in the three fundamental biological energies, or Doshas (Vata, Pitta, and Kapha), specifically focusing on the accumulation of deep-seated imbalances in the bodily tissues (Dhatus).

 

Fibroid formation is primarily seen as a disorder involving the vitiation of Vata and Kapha Doshas, along with the contamination of the Rakta (Blood), Mamsa (Muscle), and Meda (Fat/Adipose) Dhatus.

 

Vata Vitiation: Vata (air and ether) is responsible for movement and division. Its aggravation (often due to stress, irregular lifestyle, or poor diet) is considered the initiating factor that leads to abnormal growth, stagnation, and hardening of tissues.

 

Kapha & Meda Vitiation: Kapha (earth and water) is responsible for structure and bulk, and Meda (fat) is a key tissue involved. Excessive consumption of heavy, oily, sweet, or cold foods, and a sedentary lifestyle, can increase Kapha and Meda. This leads to the thickening, accumulation, and formation of the dense, knotty mass (Granthi) in the uterine muscular layer (Mamsa Dhatu).

 

Artava Vaha Srotas Obstruction: The channels (Srotas) carrying menstrual fluid (Artava) become blocked (Sroto Sanga) due to the Kapha-Meda accumulation, further contributing to symptoms like heavy bleeding and pain.

 

The symptoms of Garbhashaya Granthi largely depend on the size and location of the fibroids.

 

Symptom Category Ayurvedic Perspective

Menstrual Issues :   Asrigdara (Heavy/Prolonged Menstrual Bleeding - Menorrhagia), Kastartava (Painful Menstruation - Dysmenorrhea), Irregular Cycles.

Pain & Discomfort  Pelvic Pain, Lower Backache, Pressure, or a feeling of heaviness in the abdomen (Kukshi Gurutwam).

Fertility  Difficulty in Conceiving (Infertility), Recurrent Miscarriages.

Pressure Symptom : Increased frequency of urination (due to pressure on the bladder), or Constipation (due to pressure on the rectum).

 

Ayurveda offers a holistic, non-surgical approach for the management of fibroids (especially smaller, asymptomatic, or mildly symptomatic ones), focusing on reversing the disease process (Samprapti Vighatana).

 

The treatment aims at:

 

Vata Anulomana: Regulating the Vata Dosha to clear blockages and normalize movement.

 

Kapha & Meda Shamana: Reducing and dissolving the accumulated fibrous mass.

 

Lekhana: The 'scraping' action of herbs to reduce the size of the Granthi.

 

Rakta Prasadana: Purifying the blood and addressing excessive bleeding.

 

Strengthening the Uterus: Rejuvenating the uterine tissue (Garbhashaya).

 

Detoxification Therapies (Shodhana Chikitsa)

 

Panchakarma therapies are crucial for deep-rooted cleansing and breaking the pathogenesis.

 

Virechana (Therapeutic Purgation): Cleanses the body of accumulated toxins (Ama) and balances Pitta and Kapha.

 

Basti (Medicated Enema): The primary treatment for Vata vitiation. Specific medicated oil or decoction enemas (like Dashamoola Taila Basti) help to normalize the Apana Vata (sub-dosha governing the pelvic region) and reduce mass.

 

Uttara Basti (Vaginal Administration): Specialized therapy involving the sterile administration of medicated oil or decoction into the uterus or vaginal route, highly effective in uterine disorders.

 

C. Diet and Lifestyle Adjustments

Correcting diet (Ahara) and lifestyle (Vihara) is fundamental for long-term relief and prevention of recurrence.

 

Diet (Ahara):

 

Focus on: Fresh, light, warm, easily digestible foods. Increase intake of bitter, pungent, and astringent tastes (Kapha-reducing). Incorporate plenty of green vegetables, whole grains, and legumes.

 

Avoid: Excessive cold, oily, processed, refined, or heavy foods, dairy, red meat, and deep-fried items, as these increase Kapha and Meda.

 

Lifestyle (Vihara):

 

Maintain a regular routine and get adequate sleep.

 

Practice Yoga and Pranayama to manage stress and normalize Apana Vata. Poses like Baddha Konasana (Bound Angle Pose) are often recommended.

 

Avoid daytime sleeping (Diwaswapna), which is known to increase Kapha.
